Three doors to care

Most common

1 · A gender team

A team of psychologists and doctors. They confirm the diagnosis of gender incongruence, then hormones and surgery follow. University teams: Amsterdam UMC, Radboudumc, UMCG. There are smaller providers too.

Needed: a GP referral.

New

2 · Your GP

Since June 2025 there is guidance that lets a GP prescribe and monitor hormones for adults with capacity, without a gender team. It is not an official GP guideline: not every GP does it.

No gender team

3 · A GGD trans clinic

The public health services (GGD) in Amsterdam and Utrecht provide hormone care without a gender team referral, for specific groups. Rotterdam is next.

Route

Step by step via a gender team

  1. GP

Ask for a referral to one or more gender teams. Ask for several at once: you may be on more than one waiting list.

  2. Waiting list

Expect months to years. Check the current waiting times and keep in touch with the provider yourself.

  3. Assessment

A specialised psychologist or psychiatrist assesses gender incongruence. This is required for physical treatments to be covered.

  4. Team meeting

The team discusses with you which treatments fit.

  5. Hormones

Usually via an endocrinologist, later via your GP. See hormones.

  6. Surgery and other care

Chest, genital, face, voice, hair removal. For many procedures the hospital asks your insurer for prior authorisation.

Context

Good to know

  • The Dutch quality standard for trans care (2019) is outdated and being revised. Until then the old standard applies, plus the international WPATH SOC8.
  • Trans care is covered by basic health insurance. You do pay your deductible (eigen risico, € 385 in 2026).
  • Gender Team South Netherlands closed in December 2024. About 900 people had to re-register elsewhere.
  • The government wants to end coverage for non-contracted care. That affects many smaller providers. Keep an eye on this.
Tip from experience: a referral must be exactly right: the right department and the right doctor. Check signatures and dates. One mistake can cost you months.
