Explained

What is FFS?

Facial feminization surgery: operations on bone and soft tissue that make masculine features in the face more feminine. Think of the forehead and brow ridge, hairline, nose, jaw and chin, lips, cheekbones and the Adam's apple. Hormones change skin and fat, but not bone.

Rules

When is it covered?

Only for a passability problem: because of your masculine face you are not recognised as a woman, and people are startled, react strangely or disrespectfully. The feature must be "so masculine that it does not generally occur in women". Your doctor's request, with photos in your gender role, must answer three questions:

  1. Why is there a passability problem?
  2. Which feature contributes most?
  3. How does the procedure improve that?

The bar is high: courts and the disputes committee have rejected several cases. But under settled case law the treating doctor's judgement comes first, and in case of doubt the insurer must consult your doctor.

Rejected? The 3D page (in Dutch) has a letter for your doctor to copy, the twelve steps up to court, and case law your lawyer can cite.
